Become a Member of Comprehensive MD!
Becoming a member of Comprehensive MD of South Florida is simple and easy. As a member, you will gain access to the many services we offer.
Membership is currently only being offered to patients cared for by providers affiliated with Privia Health in the South Florida region.
You may become a patient of one of our current providers, or you may find another Provider here: https://providers.priviahealth.com/
Step 1: Send us an email ([email protected]) with the following:
- Name
- Email address
- Phone number
- Some information about yourself and what services you are looking for with Comprehensive MD
Step 2: Once your email is received and processed, one of our team members will get back to you via email or phone call as soon as possible to help get you on board. We will send patient information forms, consent forms, etc. for you to complete. If you are already a patient of a Privia provider, we may already have most of this information.
Step 3: After all of the information is received, we will plan a home visit with one of our providers of your choice.
